How to Make AppointmentTO SCHEDULE YOUR APPOINTMENT:
Please call us at (812) 234-0555 or (800) 592-4984 to schedule an appointment for MRI. Please review the following questions before calling and have a doctor's referral.
MRI Screening Questions:
* Have you ever had a MRI before?
* Have you EVER had any metal in your eye?
* Do you have a pacemaker or pacemaker wires?
* Have you ever had heart or brain surgery?
* Have you had aneurysm surgery?
* Do you have any surgical hardware left in you body?
i.e. clips, wires, etc.?
* Have you had any surgery in the past six weeks?
* Do you have any implants?
i.e. eye, ear, breast, penile, etc.?
* Do you have any metal of any kind in your body?
i.e. bullets, shrapnel, etc.?
* Do you have a neuro stimulator inside your body?
* Do you have renal failure or renal insufficiency?
If you answered yes to any of these questions, please notify our personnel when scheduling your appointment.
